All patient information and patient records maintained by Mercy Medical Center are confidential and privileged and shall not be subject to disclosure unless authorized by the patient, or required by State and Federal law. A medical record is a unique property and is owned by Mercy Medical Center, the individual health care providers and the patient. By law, the recorded document itself must be maintained by Mercy Medical Center. Medical records can not be taken outside of Mercy Medical Center except upon receipt of a subpoena or court order. Copies of a patient's medical record may not be release without the patient's written consent except under specific circumstances outlined by federal and state law. To receive a copy of your medical record Mercy Medical Center must have your written authorization. An authorization is valid for 90 days unless otherwise specified. The charge for copies of medical records is in accordance with General Licensure Standards for Health Care Facilities for the State of Colorado. The charge for obtaining a copy of the medical record will be: $14.00 for the first 10 or fewer pages $ .50 per page for pages 11-40 $ .33 per page for every additional page $ 1.50 per page for microfilmed records (any record greater than 2 years old) For more information please visit My Personal Health Record.
